Who are we?

ADULT PROTECTIVE SERVICES, In each county in North Carolina, the County Department of Social Services is mandated to provide Adult Protective Services. This service is designed to protect the disabled adult who is abused, neglected, exploited, and in need of protection as defined by North Carolina State Law. An essential component of the service is the preservation of the client’s right to self-determination recognizing the need for the least restrictive plan., , GUARDIANSHIP SERVICES, Guardianship involves the provision of services to individuals who are alleged to be incompetent. This includes legal proceedings in which the court declares the adult incompetent. Another party is then given responsibility for duties relative to the adult’s personal affairs and/or property. The guardian’s responsibility is determined by the court system, and is based on applicable provisions of the law and the circumstances of the individual case. Guardianship ends with the death of the adult, or with judicial restoration of the adult’s competency., , IN-HOME AIDE SERVICES, In-Home Aide service is a help service provided to adults to assist them in meeting everyday needs, maintaining self-sufficiency, and improving quality of life so they can remain at home. These services include assistance with personal care needs; health needs, as well as assistance with home management, budgeting, and nutrition. To be eligible for In-Home Services you must be a resident of Lincoln County, and need assistance in everyday needs., , PLACEMENT SERVICES, Placement services are activities necessary to assist aging or disabled individuals and their families or representatives in finding substitute homes or residential health care facilities suitable to their needs when they are unable to remain in their current living situations., , ADULT HOME SPECIALIST, The Adult Home Specialist is responsible for the routing monitoring of licensed facilities to ensure compliance with the rules according to General Statute 131D-2(b)(1a). The Adult Home Specialist also handles complaint investigations, corrective actions, and monitoring corrective actions of homes out of compliance., , TRANSPORTATION, Transportation services are provided through a provision of Medicaid Assistance. To be eligible for transportation, the recipient must have Medicaid. Services include transportation to and from medical appointments and pharmacies. Contact your Medicaid worker for more information., , SPECIAL ASSISTANCE/IN HOME (SA/IH), Special Assistance/In Home provides a choice for those who are facing Rest Home placement. This program provides supplemental income, which is used to help that individual meet basic needs and remain safe while living at home. To be eligible for this program the adult must have Medicaid, and their personal physician approval to participate in this program. For more information you can go to the North Carolina Division of Aging and Adult Services at North Carolina Division of Aging and Adult Services website., , COMMUNITY ALTERNATIVE PROGRAM FOR CHILDREN, The CAP-C program is a community alternatives program that offers disabled children of Lincoln County an opportunity to continue living in their own homes rather than facility placement. To be eligible for this service you must be in need of medical attention, meet the criteria for nursing home care, and/or need support services to remain safely in the home. You must be Medicaid eligible and have your personal physician’s approval to participate in this program., , , COMMUNITY ALTERNATIVE PROGRAM FOR DISABLED ADULTS, The CAP-DA program is a community alternatives program that offers the elderly and disabled residents of Lincoln County an opportunity to continue living in their own homes rather than facility placement. To be eligible for this service you must be in need of medical attention, meet the criteria for nursing home care, and/or need support services to remain safely in the home.
